What needs to be checked before a claim is prepared?

For each episode, match the order and service dates to the payer, plan, authorized service and billing entity. Record the evidence used and the information still required. A benefit verification or authorization is an input to the workflow. It does not guarantee reimbursement.

  • Identify whether the approved workflow routes each component through a pharmacy benefit, medical benefit or other payer-specific arrangement. Escalate unclear coverage or billing responsibility.
  • Compare authorization dates, quantities or visits against the supplied dispensing, delivery or service records. Route missing orders, signatures or clinical notes to the designated provider contact.
  • Track the drug, pump or supplies and any professional visit as distinct documentation items. Your billing lead determines how each is coded, grouped and submitted under the applicable payer rules.

Worked example

What if the drug delivery is documented but the visit is outside the authorization?

A pharmacy has delivery confirmation for the drug and supplies. A separately recorded nursing visit falls outside the dates shown on the current authorization. The biller logs which component is affected, requests the current authorization record and routes the visit to the client reviewer. The reviewer decides the billing treatment. The preparer does not alter dates or infer a clinical service from a delivery note.

What happens after a rejection or short payment?

Classify the payer response against the original claim reference. Route eligibility mismatches, missing authorizations, missing records and coding questions to their respective owners. For a paid line, compare the allowed amount with the applicable contract rate and distinguish patient responsibility from payer payment. Track corrected claims, open payer decisions and additional cash actually posted separately.

Can your billing team start with one queue?

Choose a payer, therapy category and one stage such as intake or authorization exceptions. Agree sample dates, access restrictions, documentation standards and who may contact the payer. Measure completeness at handoff, rework, days awaiting provider records and exceptions resolved. Use the observed effort to quote a recurring service.

Before the first live queue

Bring the inputs that shape your scope

The proposal should identify what can be assigned, who can decide, and how you will accept the work. Start with one representative queue; agree any expansion after reviewing the pilot.

  1. Define the workload

Therapy, supplier eligibility, payer and service-line mix.

  2. Agree the controls

Order, delivery and visit evidence available for the affected dates.

  3. Review the right measures

Claim-file readiness, rejection reasons and client-reviewed exceptions.

Before you decide

Buyer questions

Clarify the work, access and approval boundaries before assigning a team.

What can an infusion pharmacy hand off without transferring clinical decisions?

Actigy BPO can reconcile authorized dates, drug and supply documentation, visit records and open billing exceptions. Your billing owner approves coding and release. A designated clinician handles clinical questions.

Can you work alongside a pharmacy’s own billers?

Yes. The retained team can keep coding, claim release and payer contracting while Actigy BPO prepares a defined queue. Set a handoff owner for pharmacy records, service visit documentation and payer exceptions.

Does every payer cover the drug, equipment and home visits together?

Coverage and claim routing depend on the particular benefit, payer, product and episode. Use the current payer policy and client-approved billing instructions for each component. Your team resolves coding, benefit and clinical ambiguities.

What information is useful for a quote?

Share payer mix, monthly episode or claim-line volumes, the queue that needs help, current systems, permitted access locations and any required hours. Keep patient details out of the inquiry. Arrange controlled access to a redacted example after scoping.
